  • Student Information: Full name, address, date of birth, and contact information.
  • School Information: Name of the homeschool (e.g., “Smith Family Homeschool”), address, and contact details.
  • Academic Record: List of courses, grades, credits earned, and academic years.
  • Grading Scale: Explanation of the grading system (e.g., A=90-100, 4.0 GPA scale).
  • Cumulative GPA: Overall grade point average.
  • Credits Earned: Total credits accumulated toward graduation.
